New Request for Health Records Form: Coming Fall 2025

This fall, HIROC will be introducing a Request for Health Records Form. Subscribers will be asked to complete this new form when records are first provided to a HIROC adjuster or counsel, either for a potential or actual claim.

To help make this process as smooth as possible, HIROC will be hosting virtual information sessions to go over the process, the form, why we’re doing it, and what is being asked of Subscribers. 

HIROC is offering three dates for the session (Subscribers need only attend one):

  • October 24, 11 AM ET
  • November 5, 1 PM ET  
  • November 17, 11 AM  ET

Why Is This New Form Important?

In advance of the sessions, we sat down with Jonathan Gutman, HIROC’s Legal Counsel, to learn more about the new form and information sessions.

First off, who should attend a session? 

Subscriber risk and health records team members are encouraged to attend. If that’s not you, we encourage you to share this article and the session registration with those team members.

Why is HIROC implementing a Request for Health Records Form?

HIROC is implementing a standardized Request for Health Records Form to help ensure that its adjusters and counsel are provided with a well-organized and complete copy of all relevant health records, as early as is practical. The form really serves as a nudge to ensure we’re getting the records we need, even if they are not part of "the chart."

One thing I want to highlight is that it will also help reduce the number of requests that are made of Subscribers after a potential or actual claim is filed.

This form is all about making the process for Subscribers more efficient and reducing surprises. We know your time is valuable.

Who will need to fill in the form?

That may vary among different Subscribers, but it’s similar to those we are asking to attend the sessions. 

We recommend that the individuals directly responsible for gathering documents (such as Health Records Specialists) and individuals communicating with HIROC adjusters and counsel (like risk team members) work together on completing, verifying, and submitting the Request for Health Records Form.

Where can we find the form, and when do Subscribers need to start using it? 

We will share the form and confirm the start date for using it once the sessions have kicked off in October. No need to start today. 

Once it’s online, Subscribers will be able to find the form and a robust FAQ under HIROC.com/resources. Note that you must be logged in to access it.
