New Request for Health Records Form: Coming December 1
This fall, HIROC will be introducing a Request for Health Records Form. Subscribers will be asked to complete this new form when records are first provided to a HIROC adjuster or counsel, either for a potential or actual claim.
The form and an FAQ will go live on December 1 under the HIROC Member Portal Links drop-down on HIROC.com. Subscribers must log in to access the form and FAQ.
Why Is This New Form Important?

Why is HIROC implementing a Request for Health Records Form?
HIROC is implementing a standardized Request for Health Records Form to help ensure that its adjusters and counsel are provided with a well-organized and complete copy of all relevant health records, as early as is practical. The form really serves as a nudge to ensure we’re getting the records we need, even if they are not part of "the chart."
One thing I want to highlight is that it will also help reduce the number of requests that are made of Subscribers after a potential or actual claim is filed.
This form is all about making the process for Subscribers more efficient and reducing surprises. We know your time is valuable.
Who will need to fill in the form?
That may vary among different Subscribers, but we recommend that the individuals directly responsible for gathering documents (such as Health Records Specialists) and individuals communicating with HIROC adjusters and counsel (like risk team members) work together on completing, verifying, and submitting the Request for Health Records Form.
