Subscribers are asked to complete this new form when records are first provided to a HIROC adjuster or counsel, either for a potential or actual claim. It is intended for Subscribers that maintain health records for patients or clients.

Why has HIROC implemented a Request for Health Records Form?
This new standardized Request for Health Records Form helps ensure that HIROC adjusters and counsel are provided with a well-organized and complete copy of all relevant health records, as early as possible. The form really serves as a nudge to ensure we’re getting the records we need in the event of a claim or potential claim.
“This form is all about making the process for Subscribers more efficient and reducing surprises. We know your time is valuable,” shared Jonathan.
Who needs to fill in the form?
This varies among different Subscribers, but we recommend that the individuals directly responsible for gathering documents (such as Health Records Specialists) and individuals communicating with HIROC adjusters and counsel (like risk team members) work together on completing, verifying, and submitting the form.
As a reminder, only Subscribers that maintain health records for patients or clients need to use the form.

The form and an FAQ went live as of December 1 under the Member Portal Links on HIROC.com (log in required).
