HIROC Risk Management Advisory Team
HIROC Vision: “Partnering to create the safest healthcare system.”
HIROC’s Risk Management team draws upon a wealth of diverse knowledge along with extensive experience in all aspects of healthcare risk management. Beyond our own expertise, we rely on current risk management resources on leading practices and patient safety to assist in supporting our members and subscribers. Our collaborative team approach provides subscribers and members with timely responses to queries or other advisory requests. Strong advocates for a “safer healthcare system”, the risk management team works with our members and subscribers to ensure we achieve this vision.
The following individuals are the current members of HIROC’s Risk Management Advisory Team. This profile is meant to provide the member with an indication of the qualifications and identity of the Risk Management Advisory team. The team will evolve over time as our members grow, new members join, responsibilities change and new teammates are added.

Polly Stevens brings a wealth of experience in the areas of healthcare risk management and quality improvement. Prior to joining HIROC, Polly served as Director, Quality and Risk Management at the Hospital for Sick Children in Toronto, where she led the development and implementation of a number of programs, including the nationally and internationally recognized SickKids Blueprint for Patient Safety. Previously she was the Director, Quality and Risk Management at North York General Hospital. Polly also serves as Adjunct Professor in the Department of Health Policy, Management and Evaluation at the University of Toronto.
Polly earned a Masters of Health Science (Health Administration) from the University of Toronto and a Bachelor of Administrative Studies (Honours) from York University. She has completed the Patient Safety Leadership Fellowship of the American Hospital Association and the Advanced Health Leadership Program of the Rotman School of Management. In 2007, the University of Toronto Society of Graduates in Health Policy Management and Evaluation presented Polly with the Leadership Innovation Award for her work in patient safety. |

Arlene began her career as a Certified Health Information Management Professional. Arlene holds a Bachelor of Science degree with a major in Genetics and in 2006 became the first resident Canadian to successfully write the CPHRM (Certified Professional in Healthcare Risk Management) exam, sponsored by the American Hospital Association. Arlene has extensive experience in Risk, Quality and Health Information Management as well as Patient Safety and Client Service. Arlene is the editor of HIROC’s quarterly newsletter – The HIROC Connection, represents HIROC on the Advisory Committee of the Ontario Node of safer healthcare now! and is Treasurer of the Canadian Healthcare Risk Management Network.

Kathryn Callfas, B.A.
Manager, Risk Management Member Services
Kathryn worked for eighteen years in various healthcare settings as a Certified Pedorthist. She holds a Bachelor’s degree in Physical Education,
and has successfully completed the Sunnybrook/University of Toronto-Rotman Health Policy Management and Evaluation Leadership Development Program.
She has a diverse background in healthcare with many years of leadership and management experience in settings that include a large teaching hospital
and private outpatient clinics.
Kathryn led the planning, implementation and evaluation of the RMSAM™ program at Sunnybrook Health Sciences Centre in
2006 and is currently pursuing her Certified Risk Management designation.

Mitra has a background in civil litigation, insurance defense and healthcare risk management. She also has advised on U.S. immigration matters for health professionals. Prior to joining HIROC, she worked in risk management for a multi-hospital network in the U.S. Mitra obtained a dual B.A. in Public Policy and Sociology from Brown University and a Juris Doctor from Boston University School of Law. She is a member of the Bar of Massachusetts. Mitra is the principle specialist responsible for contract and waiver review.

Alex holds a Bachelor of Science in Occupational Health & Safety from Ryerson University in Toronto and completed the Canadian Healthcare Association’s Risk Management and Safety in Health Services course in 2007. Prior to joining HIROC, he worked in Occupational Health & Safety for various industries including construction, government and healthcare. Alex was responsible for the research and development of the RMSAM™ Facilities & Engineering Module. He is the principle risk management specialist involved in issues related to fraud.

Kopiha holds a Bachelor of Commerce in Information Technology Management with a major in System Analysis and Design and completed the Canadian Healthcare Association’s Risk Management and Safety in Health Services course in 2007. Prior to joining the HIROC, Kopiha had information technology related experience from various industries including government, retail and insurance. Kopiha has been seconded to the Insurance and Claims system implementation project to replace the current system, enhance the current reporting capabilities and represent subscriber/member needs.

Eleanor has a broad background in healthcare, in Canada and Australia, with a number of years dedicated to work in the fields of health information management, research, patient placement, quality improvement, risk management and patient safety. She holds an Honours Bachelor of Administrative Studies in Health Administration from York University. Eleanor spearheaded the expansion of HIROC’s risk management services including the development of RMSAM™ (Risk Management Self-Appraisal Modules) and has been instrumental in moving HIROC forward with its vision of “Partnering to create the safest healthcare system”. Eleanor is a Director of the Institute for Safe Medication Practices (ISMP) Canada, represents HIROC as a voting member of the Canadian Patient Safety Institute (CPSI) and sits on The Patient Safety Roundtable jointly hosted by CPSI and Accreditation Canada. |

Sara has an undergraduate degree in biology and psychology, and is currently enrolled in classes at the Insurance Institute for the CIP designation program. Sara’s previous experience included roles related to federal taxes and statistics. Sara is the web-master for the Canadian Risk Management Community Forum and supports the production and distribution of The HIROC Connection and HIROC’s Risk Management Claims Analysis reports.
